Create an audit receipt for the current agent run, PR review, MCP change, or tool-call workflow.

Goal:
Make the result falsifiable. Separate the outcome from the evidence quality so a human can see what was checked, skipped, unobservable, or incomplete.

Process:
1. Identify the exact workflow or run being audited.
2. List the claims the run made, such as reviewed files, tools called, policies enforced, tests run, redactions applied, or approvals checked.
3. For each claim, record the concrete evidence source.
4. Mark missing, weak, or unobservable evidence as incomplete instead of silently trusting it.
5. End with a verdict that distinguishes outcome from audit coverage.
